The information comes from a variety of sources including a worksheet the mother fills out, the mother's prenatal record, and the delivery record. The Office of Vital Records and Statistics has a Quality Control program where every hospital is audited annually. Births are randomly selected and hospital records are checked to verify the accuracy of the reported information. Overview The birth data in this IBIS-Q query module come from the Utah Birth Certificate database. The database is maintained by the Utah Department of Health, Office of Vital Records and Statistics. The population data used for the denominator were produced by the Utah Governor's Office of Planning and Budget (GOPB) for years 1980-1999 and the U.S. Census Bureau for years 2000 and later, are considered the most accurate available. Getting Started Follow the steps to narrow your query and display your results. When you are done forming your query, click "Submit" bottom of the screen (last step) to get your query result.
