In 2015 the San Juan County Local Health District was formed. Data reported are for all years using the current boundaries. BirthData Birth certificates are filed electronically by hospital birth certificate clerks. The information comes from a variety of sources including a worksheet the mother fills out, the mother's prenatal record, and the delivery record. The Office of Vital Records and Statistics has a Quality Control program where every hospital is audited annually. Births are randomly selected and hospital records are checked to verify the accuracy of the reported information. Overview The birth data in this IBIS-Q query module come from the Utah Birth Certificate database. The database is maintained by the Utah Department of Health, Office of Vital Records and Statistics. "Utah Small Areas" refers to a set of geographic areas in Utah with population sizes ranging from approximately 16,000 to 80,000 persons. These geographic areas are especially useful for doing public health assessment in communities within Utah's urban counties.
