BirthData
Birth certificates are filed electronically by hospital birth certificate clerks. The
information comes from a variety of sources including a worksheet the mother fills out, the
mother's prenatal record, and the delivery record. The Office of Vital Records and Statistics
has a Quality Control program where every hospital is audited annually. Births are randomly
selected and hospital records are checked to verify the accuracy of the reported information.
Mort
Death certificates in Utah are required to be filed by funeral directors. Funeral directors obtain demographic information from an informant, a close family member of the decedent. The cause of death is certified by the decedent's physician or the physician that attended the death. Accidental and suspicious deaths are certified by the Medical Examiner. Death certificate data go through extensive edits for completeness and consistency. The Office of Vital Records and Statistics (OVRS) does annual trainings for funeral directors and local registrars.
When death certificates are received the cause of death literals are keyed into software locally by Office of Vital Records and Statistics (OVRS), then shipped to NCHS where they are machine coded into ICD-10 codes. NCHS returns the ICD-10 codes to OVRS where the death records are updated.
For rates where the count is zero, a numerator of "3" was used to calculate the confidence interval (per Lillienfeld and Stolley, Foundations of Epidemiology, 1994).
Overview
The death data in this IBIS-Q query module come from the Utah Death Certificate database.
The Utah Department of Health and Human Services, Office of Vital Records and Statistics maintains the database.
Infant mortality is the number of infants who died before they were 365 days old (from 0
through 364 days of age).
Birth-Death Linked Infant Mortality (Birth Cohort) provides the number of infant
deaths that occurred among infants born during a particular year (regardless of
the year of death). This module gives the number of infant deaths among infants
born in a given calendar year.

Small for gestational age (SGA) is a term used to describe a baby who is smaller than the usual amount for the number of weeks of pregnancy. SGA babies usually have birthweights below the 10th percentile for babies of the same gestational age. This means that they are smaller than 90 percent of all other babies of the same gestational age.
Large for gestational age (LGA) is a term used to describe a baby who is larger than the usual amount for the number of weeks of pregnancy. LGA babies usually have birthweights above the 90th percentile for babies of the same gestational age. This means that they weigh more than 90 percent of all other babies of the same gestational age.

A tertiary care facility is a hospital with a unit that has continuously available personnel and equipment to provide life support for as long as needed. Tertiary care facilities are able to provide care to newborn infants with birthweights less than 1000 grams or gestational ages less than 28 weeks.
